How to Prepare for your First Visit
to Maui Facial and Dental Surgery
We are located in downtown Wailuku and on the second floor of the Wailuku Executive Center building. It is a 4 story white stucco building with mirrored windows and a red tile roof.
Parking for our office is reserved for our patients and only our patients in a lot next to our building and accessed by Main St. It is located at the intersection that also borders the Texaco gas station and the Good Shepherd Episcopal Church. The parking lot shares a space with the Main St. Food Truck. Our parking stalls are designated with signs for your convenience.
What To Bring To Your Appointment:
- Your surgical referral slip and any x-rays if applicable.
- A list of ALL medications you are presently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
What Your Initial Visit will Consist Of:
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.
